I would say the 1981 Cleveland Browns, who I know finished in last place and somehow beat both the Niners & Bengals that year. I believe that they are the only team to defeat both NFL finalists in the same season and finish in last place.
I'm pretty sure they had P Steve Cox on that team, who handled KO duties and would go on to make a 60 yd FG later in his career.
As for the rest of the criteria, I am not certain.
