Carl Madsen, a replay official and former umpire, died at 71 en route home from the Chiefs-Titans game. The last time an actively scheduled official died during the season was David Hamilton, who passed days before he was to work as an alternate official in the 1994 conference championships.

Thanks for posting. I remember that also during the 1994-95 playoffs, field judge Donnie Hampton also died. Other officials I remember who passed away during the season were Vince Jacob (1982) and Ray Douglas (1983).
