It's not unusual for a player to make a touchdown while being tackled at the same time but I never saw any player ended up laying on the football field injured afterwards.

It happened to Bob Windsor in a 1974 Patriots vs Vikings game.

I believe that happened to Charley Taylor in 1971. He was tackled from behind while scoring a TD against the Chiefs (I think), and broke his ankle on the play. But he might've been tackled just short of the goal line, in which case this wouldn't qualify.

In 1975, the Browns' Billy Lefear came within eight yards of doing it after running a kickoff back 92 yards. He never played in an official game again.

In 1986, Buffalo WR Jerry Butler went up over two Miami DB's to grab a touchdown pass from Jim Kelly, but when the came back down, he suffered a broken leg. Not a regular tackle, but the defenders did unintentionally cause the injury, which ended Butler's career.

In 1979 Drew Pearson got injured after celebrating a TD against the Giants. I bet Tom Landry wasn't very pleased, even that it was Pearson's third TD catch of the game. He missed his first career game the following week.

I was at that game and remember it vividly. You could hear a pin drop in the stadium. Jerry Butler was a fan favorite.
